29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    a)  Traffic Control West, Inc. at 23rd Ave. and 2nd Street, Greeley, CO 80631:  On or before November 4, 2015, the employer did not instruct each flagger in the recognition and avoidance of unsafe conditions specific to this work environment, and the actions necessary to eliminate, control, and mitigate the associated hazards.  The unsafe conditions included:     1)  The volume of traffic;    2)  The extension of the work period into the night;    3)  The inability of the flaggers to communicate in the dark; and    4)  The positioning of the flaggers outside the active traffic lanes prior to stopping traffic.    Abatement note:  The instruction failed to include the movement of employees, equipment, and materials in  the work zone; the duration of work; the traffic control plan; and the communication between flaggers and between the flaggers and the traffic control supervisor.

29 CFR 1926.56(a): While work was in progress, construction areas, ramps, runways, corridors, offices, shops or storage areas were not lighted to the minimum illumination intensities as listed in Table D-3 of 29 CFR 1926.56:  a)  Traffic Control West, Inc. at 23rd Ave. and 2nd Street, Greeley, CO 80631:  On or before November 4, 2015, while work was in progress, the construction area was not lighted to the minimum illumination intensities as listed in Table D-3 of 29 CFR 1926.56.  Flaggers were directing traffic at flagger stations set up at the northbound and southbound lanes of 23rd Avenue, and at the westbound lane of 2nd Street.  The sewer bypass company was connecting pipes and a pump to the existing sewer line that was accessed through the manhole located at this intersection.  The sunset was at 4:52 pm., and at 6:39 pm, a flagger was struck by a vehicle travelling in a 35 mph speed zone.  The work area was not provided with at least 5 foot candles of illumination.  Condition exposed employees to the hazard of being struck by motor vehicles, resulting in serious bodily injury.
