29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  At the time of the inspection, employees in the Ball Mill area were found to be exposed to noise above the 85 decibel threshold for an 8 hour average;  a) A Vacuum mill employee was sampled for 238 minutes and received an average exposure of 89.8 decibels.  B) A Screener near the Ball mill was sampled for 246 minutes and received an average exposure of 89.7 decibels.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Employees in the lab were working with acids and bases without an eyewash readily available.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  At the time of the inspection, four pinch points on the main furnace were not guarded. When the furnace was rotated for casting the west pinch points were ingoing. When the furnace was rotated back to upright the east pinch points were ingoing.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment;  At the time of the inspection, the employer had not conducted complete PPE assessments for employees working with hazardous chemicals such as:  1. Silver casting in furnace room.  2. Screening of dross containing heavy metals.  3. Screening of dross with nuisance dust.   4. High noise in areas adjacent to the Ball Mill.
