29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or a personal fall arrest system:    a)    On or about November 20, 2015 the employer exposed employees to fall hazards of at least 11 feet 3 inches, in that fall protection was not provided while engaged in roofing activity on a 3:12 low-slope commercial roof.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503(a)(2)(i) through (viii):   a)    On  or about November 20, 2015, the employer exposed employees to fall hazards of at least 11 feet 3 inches, in that fall-protection training was not provided by a competent person before allowing employees on a 3:12 low-slope commercial roof to complete a roofing activity.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     a)    On or about November 20, 2015, the employer exposed employees to fall hazards of up to 11 feet 3 inches, in that a 20-foot portable ladder side rails were not extended at least 3 feet above the upper-landing surface of a 3:12 low-slope commercial roof.
