The employer had not complied with the provisions of 29 CFR 1926.652(b)(1) On or about November 23, 2015, at a worksite near the intersection of Nina Rd. and Tyler Run Blvd. in Conroe, TX, two employees were exposed to a cave-in hazard when installing water lines in a trench exceeding a depth of five feet without an adequate protective system.
