29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On or about November 20, 2015 and at times prior thereto, at or near 1192 East Henri De Tonti Blvd. Tontitown, Arkansas, employees used pneumatic nail guns, circular saws and hand tools such as hammers without eye protection. This condition exposed employees to eye injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    On or about November 20, 2015 and at times prior thereto, at or near 1192 East Henri De Tonti Blvd. Tontitown, Arkansas, employees were working at heights of 18 feet above the ground below performing job tasks such as but not limited to framing and roofing. This condition exposed employees to the hazard of falling from heights.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about November 20, 2015 and at times prior thereto, at or near 1192 East Henri De Tonti Blvd. Tontitown, Arkansas, employees who routinely worked at heights as a routine part of their job while performing job tasks such as but not limited to framing and roofing, had not received training in fall protection. This condition exposed employees to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about November 20, 2015 and at times prior thereto, at or near 1192 East Henri De Tonti Blvd. Tontitown, Arkansas, employees used metal extension ladders to access the roofs and framing of the building under construction. These ladders did not extend at least three feet above the landing surface, exposing employees to fall hazards.
