29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operators and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) J-M Farms, Inc. dba Rocky Mountain Mushrooms at 415 Yuma Street, Denver, CO 80223: On and before November 16, 2015, the employer did not ensure that the gears between the electric motor and the cutting blades mechanism on the Slicer Machine were provided with an adequate guard to protect operators and other employees from hazards created by ingoing nip points. This condition exposed the employees in the Slicer Room to hazards associated with unprotected ingoing nip points that resulted in an amputation for one employee. (b) J-M Farms, Inc. dba Rocky Mountain Mushrooms at 415 Yuma Street, Denver, CO 80223: On and before November 16, 2015, the employer did not ensure that the motor shaft that drives the cutting blades mechanism on the Slicer Machine was provided with an adequate guard to protect operators and other employees from hazards created by rotating parts. This condition exposed the employees in the Slicer Room to hazards associated with unprotected rotating parts.
