29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. On or about November 20, 2015 and at times prior thereto, at the worksite located at or near 1192 East Henri De Tonti Blvd, employees of Sabino's Construction were using a jobsite made scaffolding that was attached to a powered industrial truck to allow employees access to framing. This condition exposed employees to fall hazards.

On or about November 20, 2015 and at times prior thereto, at or near 1192 East Henri De Tonti Blvd., Tontitown, Arkansas, employees used pneumatic nail guns and hand tools such as hammers without eye protection. This condition exposed employees to eye injuries.

On or about November 20, 2015 and at times prior thereto, at the worksite located at or near 1192 East Henri De Tonti Blvd., Tontitown, Arkansas, employees who were performing framing construction on the second story of a multi family construction, were working from a jobsite built scaffolding that did not have guardrails on all sides, exposing employees to fall hazards.
