29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about November 23, 2015, on the west side of the roof, where employees were exposed to a fall hazard of more than 15 feet above a lower level when performing decking work without a fall protection system. The Hercules Steel Erectors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.760 (a)(1), which was contained in OSHA inspection number 315195446, citation number 2, item number 1 and was affirmed as a final order on May 19, 2011, with respect to a workplace located at 16626 FM 2920, Tomball, Texas 77377.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about November 23, 2015, on the east side of the building, where employees were exposed to fall hazards when using an extension ladder with the ladder side rails that did not extend at least 3 feet above the upper landing surface. The Hercules Steel Erectors was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053 (b)(1) which was contained in OSHA inspection number 315195446, citation number 1, item number 1 and was affirmed as a final order on May 19, 2011, with respect to a workplace located at 16626 FM 2920, Tomball, Texas 77377.
