29 CFR 1910.23(d)(1)(iv): Stairway(s) more than 44 inches wide, but less than 88 inches wide having both sides open, did not have one stair railing on each side:    a) At the facility, west-end loading dock, on or about November 19, 2015, and at times prior thereto, the employer exposed employees to trip hazards and falls of up to 34 in. to the ground, in that a 5-riser stairway on the loading dock was not provided with adequate standard railings for employees who accessed and egressed the loading dock platform.      Recycling Revolution, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.23(d)(1)(iv), which was contained in OSHA inspection number 988653, citation number 1, item number 2, and was affirmed as a final order on March 9, 2015, with respect to a workplace located at 4190 Collins Road, Unadilla, GA 31091.

LPG tank positioned outside for employees who were expected to use fire extinguishers to fight incipient propane fires.      Recycling Revolution, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.157(c)(1), which was contained in OSHA inspection number 1010863, citation number 1, item number 4a, and was affirmed as a final order on June 6, 2015, with respect to a workplace located at 4190 Collins Road, Unadilla, GA 31091.
