Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations shall be installed so as to prevent moisture or water from entering and accumulating within the enclosures and shall be mounted so there is at least 6.35-mm (0.25-in.) airspace between the enclosure and the wall or other supporting surface. However, nonmetallic enclosures may be installed without the airspace on a concrete, masonry, tile, or similar surface. The enclosures shall be weatherproof in wet locations.     a. Located in the New York Room at 961 Thurman Road in Oak Hill, Ohio, On or about November 20, 2015 and at times prior, the installation of a 120 volt duplex receptacle enclosure used to supply electric for a scalder machine did not prevent moisture from entering the enclosure while in use, exposing employees to an electrocution hazard.      b. Located in the New York Room at 961 Thurman Road in Oak Hill, Ohio, On or about November 20, 2015 and at times prior, the installation of a 240 volt receptacle enclosure used to supply electric for a picker machine did not prevent moisture from entering the enclosure while in use and not in use, exposing employees to an electrocution hazard.

Located in the New York Room at 961 Thurman Road in Oak Hill, Ohio: an approximately three foot piece of flexible metal conduit was used to supply 120 volts from the control panel to a water valve on the side of the picker machine's drum. The metal conduit was not approved for use in damp and wet environments, exposing employees to an electrical shock hazard.
