Falls Creek Powdered Metals, Inc., 82 Industrial Park Drive, Brockway, PA 15824-Rust preventative tank area-On or about November 23rd, 2015 and times prior thereto:  The employer failed to provide a place of employment free from the recognized hazard of a struck-by hazards from potential accidental disengagement of a load due to a missing safety latch on a hook.  The hook was associated with a 1-Ton Coffing brand chain hoist (Model: JLC; Serial # JM6126ZW) in the Rust preventative tank area of the production portion of the facility.
