R&S Foaming, LLC, 205 W 10th Street, Erie, PA 16501, company pickup truck, on or about November 17, 2015 and times prior thereto: The Amerex 5 lb. multi-purpose dry chemical fire extinguisher (M/N: B402, S/N: SY-843184) had its last annual fire extinguisher inspection in December 2002. B.) R&S Foaming, LLC, 205 W 10th Street, Erie, PA 16501, company trailer, on or about November 17, 2015 and times prior thereto: The Buckeye 2.5 lb. multi-purpose dry chemical fire extinguisher (M/N: 2.5 SA ABC, S/N: AS-925753) has not had an annual fire extinguisher inspection. B.) R&S Foaming, LLC, 205 W 10th Street, Erie, PA 16501, on or about November 17, 2015 and times prior thereto: Fire extinguishers including, but not limited to, an Amerex 5 lb. multi-purpose dry chemical fire extinguisher (M/N: B402, S/N: SY-843184) and a Buckeye 2.5 lb. multi-purpose dry chemical fire extinguisher (M/N: 2.5 SA ABC, S/N: AS-925753) has not been inspected on a monthly basis.
