29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. a-Building Exterior: An employee working from a ladder jack scaffold was exposed to fall hazards of 20ft to the ground below as a means of scaffold access/egress was not utilized. Condition noted on or about 11/24/2015.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. a- Building Exterior : An employee engaged in roofing work was exposed to fall hazards of 20 ft as a guardrail system or other means of fall protection was not present. Condition noted on or about 11/24/2015 Kubamax Corp was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.451(g)(1): which was contained in OSHA Inspection Number 242953, Citation # 1, Item # 1, issued on 03/29/2012 with respect to a work place located at 48 Huron Ave, Clifton NJ 07015 which became a final order on 09/12/2012.
