29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about November 20, 2016, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances: 1) At the roll ramp in the MR7 area, a roll ramp operated by hydraulics had a missing guard. 2) At the stitching machine HT 148 area, a book flipping machine with moving parts inside was not provided with interlock device. 3) In the machine shop, a shear was operated without having a blade guard. 4) In the machine shop, a manual lathe machine was operated without having a chuck guard. 5) At the Mosca strapping machine area, a conveyor belt being used without guards for the in-going nip points. Employees were exposed to amputation and crushing hazards.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about November 20, 2016, and at times prior thereto, in press maintenance shop employees were using an abrasive wheel grinding machines where the opening between the tongue guard and the wheel was greater one-fourth inch. Employees were exposed to struck by hazards.

29 CFR 1910.219(p)(1): All power-transmission equipment shall be inspected at intervals not exceeding 60 days and be kept in good working condition at all times. On or about November 20, 2016, and at times prior thereto, the employer did not document its inspection of the pulleys and wheels associated with the pocket feeding machines every 60 days to ensure that all guards were present.
