29 CFR 1910.1000(c): The employer did not ensure that an employee's exposure to any substance listed in Table Z-3 in any 8-hour work shift of a 40 hour work week did not exceed the 8-hour time weighted average limit given for that substance in the table. (a) EQT RSM #118 Pad, Bridgeport, WV: An employee was conducting cleanup operations around the sand trailers during a gas well fracking operation and he was exposed to silica dust (respirable fraction) at an 8-hour Time Weighted Average exposure of 0.57 mg/m3 (milligrams per cubic meter of air), which was 3.6 times the 0.159 mg/m3 permissible exposure limit for a silica sample containing 61 percent quartz, over the 456 minute sampling period on November 27, 2015. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): (a) EQT RSM #118 Pad, Bridgeport, WV: Feasible engineering and/or administrative controls were not implemented while an employee was conducting cleanup operations around the sand trailers during a gas well fracking operation.
