29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered. On or about November 24, 2015, while installing lighting on a roof located at 35 Fox Trace, Hudson, Ohio 44236, the employer failed to protect employees from a fall of approximately 20 feet by using personal protective equipment.
