29 CFR 1910.134(k)(5): Retraining shall be administered annually, and when the following situations occur:  29 CFR 1910.134(k)(5)(ii): Inadequacies in the employee's knowledge or use of the respirator indicate that the employee has not retained the requisite understanding or skill; or  a.  At the work site, the employer required employees to wear N95 disposable dust masks and had trained employees about the proper wearing of the disposable dust masks.  At the time of inspection, two employees in disassembly were observed incorrectly wearing the dust mask so that only the mouth was covered while the nose remained uncovered.

29 CFR 1910.1025(c)(1): The employer shall assure that no employee is exposed to lead at concentrations greater than fifty micrograms per cubic meter of air (50 ug/m(3)) averaged over an 8-hour period.  a.  At the work site located at 727 Jenkins Avenue, Columbus, Ohio, the employer had not assured that employees were not exposed to lead at a concentration above the Permissible Exposure Limit of 50 micrograms/cubic meter in an 8-hour period.  An employee who worked in the glass loading area was exposed to lead at 5.600 mg/m3 during sampling on December 4, 2015, with zero exposure assumed for the unsampled period of time, 10 minutes.
