Keystone Automatic Technologies, Inc. - On or about 11/23/15, and at times prior there to, employees were performing servicing and maintenance work on machines and equipment that included, but was not limited to, the dust collector, the crane, CNC machines, and Baird Machines.  A lockout/tagout program consisting of procedures, employees training and periodic inspection was not developed, documented, and implemented.
