29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels:  a) Screw Department - Violation observed December 16, 2015.  A shift leader machinist operating four of seven screw machines side by side was exposed to continuous noise at 89.8% of the permissible daily dose (8-hour time weighted average sound level (TWA) of 90dBA) or equivalent sound level of approximately 89.2 dBA during the 388-minute sampling period.    b) Screw Department - Violation observed December 16, 2015.  A machinist operating three of seven screw machines side by side was exposed to continuous noise at 78.32% of the permissible daily dose (8-hour time weighted average sound level (TWA) of 90dBA) or equivalent sound level of approximately 88.2 dBA during the 388-minute sampling period.    The employer did not obtain annual audiograms on aforementioned employees as part of the company's hearing conservation program.  Employees received last annual exam on or about March 2014.
