29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Johns Manville, located at 6050 North River Road, Waterville, OH 43566: On or about November 17, 2015, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d) and required for the control of the hazardous energy sources such as, but not limited to, electrical were followed before employees conducted activities such as, but not limited to, cleaning the Base Fiber Feeder's sensor controlling the speed of the lower conveyor. Employees were exposed to amputation hazards while all hazardous energy sources such as, but not limited to, electrical were not controlled prior to conducting work.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A) Johns Manville, located at 6050 North River Road, Waterville, OH 43566: On or about November 17, 2015, the employer did not implement specific procedures to control hazardous energy of the equipment such as, but not limited to, the Base Fiber Feeder. Employees did not control all hazardous energy sources such as, but not limited to, electrical before performing activities such as, but not limited to, cleaning the sensor controlling the speed of the Base Fiber Feeder's lower conveyor. Employees were exposed to amputation hazards.
