29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (a) As observed on or about 11/25/2015, at 6112 Benjamin Road in Tampa (FL), in that, a program was not implemented for employees using half-face filtering face piece for particulates (dust mask). Employees were required to use the respiratory protection while cleaning dust and soot residue from an electrical fire without being medically qualified, fit tested and trained prior to initial use.
