29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder:    a. On November 23, 2015, on the south west end of the building, the ladder used to access the 2nd level did not extend 3 feet above the landing, exposing employees to a 12 feet fall hazard.    b. On November 23, 2015, on the south west end of the building, the ladder used to access the 3rd level did not extend 3 feet above the landing, exposing employees to a 12 feet fall hazard.

On November 23, 2015, on the west area of the 2nd level, there were several sections of the building without walls or guardrails, exposing an employee to a 12 feet fall hazard.    Florida Atlantic Iron Works was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 315498303, citation number 1, item number 1 and was affirmed as a final order on May 25, 2011, with respect to a workplace located in Orlando, Florida.

On November 23, 2015, on the west area of the 3rd level, there were several sections of the building without walls or guardrails, exposing an employee to a 25 feet fall hazard.      Florida Atlantic Iron Works was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.760(a)(1), which was contained in OSHA inspection number 316306562, citation number 1, item number 1 and was affirmed as a final order on February 29, 2012, with respect to a workplace located in Kissimmee, Florida.
