29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger operating cycle(s): (a)Twin City Die Castings Company @ 122 Cessna St. NW, Watertown, SD 57201: On or about November 18, 2016, the employer did not ensure that employees using the Denison Multi-Press #2242 were not exposed to point of operation hazards. Employees were hanging a pneumatic air nozzle on one of the two hand control levers which caused the press to actuate inadvertently. On November 17, 2015, an employee partially amputated an index finger and injured his middle finger as a result of the trim press actuating unintentionally. This condition exposed employees to an amputation hazard, in that, fingers were exposed to the point of operation due to the placement of the air nozzle on one of the control levers which caused the press to actuate.
