29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:     Warehouse, electric order picker powered industrial vehicles: employees retrieving parts from inventory racking systems were exposed to fall hazards of approximately 20 feet from the highest level where an adequate personal fall arrest system was not provided for use.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
