On or about November 19, 2015 and at times prior thereto, at the construction site located at or near 1091 E. Henri De Tonti Blvd, employees of Gabriel Quinonez used a metal extension ladder as a crawling board. This condition exposed employees to fall hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about November 19, 2015 and at times prior thereto, at the construction site located at or near 1091 E. Henri De Tonti Blvd, employees of Gabriel Quinonez worked at heights greater than 6 feet above the ground below without fall protection. This condition exposed the employees to the hazards of falls to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about November 19, 2015 and at times prior thereto, at the construction site located at or near 1091 E. Henri De Tonti Blvd, employees of Gabriel Quinonez used a metal extension ladder to access the roof that did not extend at least three feet above the upper landing surface. This condition exposed employees to fall hazards.
