29 CFR 1910.23(a)(8): Every floor hole into which persons could accidentally walk were not guarded: On or about 11/25/15, an employee engaged in maintenance activities in the Beach Activity Center (BAC) was exposed to fall hazards. The employee was exposed in that the hole/return air chase in the utility room was not guarded.
