29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface: JD Canopy LLC project site located at N205 Military Rd. Sherwood, WI: On 12/1/15, employees were exposed to falls while climbing the guardrails of the scissor lift to reach the top of the canopy which was >5' above the platform of the lift.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. JD Canopy project site located at N205 Military Rd. Sherwood, WI: On 12/1/15, employees were exposed to falls while installing steel ceiling panels on a fuel island canopy without fall protection.
