29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels: a) Eastern Wood Products: An employee, Moulder Machine Operator, was exposed to continuous noise at 38.8% of the permissible daily noise exposure limit during sampling performed for 116 minutes on January 20, 2016. For the full 8 hour work shift, the employee would be exposed to the equivalent of an average sound level of approximately 89.3 dBA. The employer did not provide annual audiometric testing for those employees exposed over the action level of 85 dBA. ABATED DURING INSPECTION NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.95(i)(3): Employees were not given the opportunity to select their hearing protectors from a variety of suitable hearing protectors provided by the employer: a) Eastern Wood Products: An employee, Moulder Machine Operator, was exposed to continuous noise at 38.8% of the permissible daily noise exposure limit during sampling performed for 116 minutes on January 20, 2016. For the full 8 hour work shift, the employee would be exposed to the equivalent of an average sound level of approximately 89.3 dBA. The employer did not provide those employees exposed over the action level of 85 dBA with muffs as an alternative to the supplied plugs. ABATEMENT CERTIFICATION REQUIRED
