29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     The employer failed to protect employees from potential point of operation/amputation hazards associated with operating an inadequately guarded drill press.  This was most recently evident during the inspection of the facility located at 600 E. Commerce Road, Lexington, NE, where the employer has employees using an unguarded Jet model OR-1758F drill press in the production area, exposing employees to fast rotating parts and potential amputation hazards.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields:    The employer is failing to protect employees from welding hazards associated with unprotected employees working in close proximity to active welding operations.  The most recent occurrence was evident during the inspection of the facility located at 600 E. Commerce Road, Lexington, NE; where the employer has an unprotected employee working near are passing by employees welding without welding curtains.  Instances include but are not limited to:                 i.  Employee welding a frame located in the production floor area, adjacent to a heavily traveled forklift path, without having installed welding curtains.                   ii.  Employees welding in their welding stations without having put up the welding curtains to protect those employees who are walking or riding forklifts down the aisle.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were not maintained in a legible condition:   The employer is failing to protect employees from struck by hazards associated with operating a powered industrial truck with illegible data plate.  This most recent occurrence of this took place during the inspection of the facility located at 600 E. Commerce Road, Lexington, NE, where the employer has employees operating a Caterpillar propane powered sit down forklift model number 21C30, serial number A2L0860112 used to move product and material around the production area of the facility without having a legible data plate.
