29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: The employer is failing to protect employees from electrical shock and amputation hazards. The companys written specific procedures were incomplete in that they failed to detail the procedures from in service through the lock-out/tag-out actions to bringing the equipment back into service. Failure to have these procedures detail the whole process could lead to employees skipping steps that could lead to serious injury or even death.
