29 CFR 1910.147(c)(4)(ii):  The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:   The employer is failing to protect employees from electrical shock and amputation hazards.  The companys written specific procedures were incomplete in that they failed to detail the procedures from in service through the lock-out/tag-out actions to bringing the equipment back into service.  Failure to have these procedures detail the whole process could lead to employees skipping steps that could lead to serious injury or even death.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   The employer is failing to protect employees from amputation and electric shock hazards.  The employer is failing to conduct inspections/evaluations of the lock-out/tag-out program.  Failure to conduct these inspections precludes the employer from identifying potential lapses in the program including, but not limited to, inadequate written lock-out tag-out program.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:   The employer is failing to ensure that all employees, who perform maintenance, are adequately trained in regards to this standard.  Employees that use lock-out/tag-out have not received not received instruction on the equipment specific procedures for Lock-out/tag-out due to the company not developing these procedures.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    The employer is failing to ensure that the tongue guard of the bench grinder is properly adjusted to protect employees from laceration hazards.  The tongue guard on the grinder in question was not correctly adjusted at the time of the inspection.
