29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a)     South Garage Shop:  A relocatable power tap (surge protector) was plugged into an extension cord which powered Milwaukee battery chargers and a florescent shop light. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.      b)     Outside Maintenance Shop:  A relocatable power tap (surge protector) was plugged into another relocatable power tap (surge protector) which powered a Haier refrigerator and DeWalt battery chargers. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.      c)     Construction/Design cubicle in Executive Suite:  A Fellows surge protector was plugged into another Fellows surge protector which was powering a laptop. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.      d)     Southwest Office in Finance:  A white surge protector was plugged into a yellow surge protector.  A Holmes heater was plugged into a surge protector. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances. This exposes employees to possible electrical fire hazards.      e)     Income Audit cubicle 3 down from the window:  A white surge protector was plugged into another white surge protector.  The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.      f)       Tribal Lottery Systems Room 114:  A refrigerator and West Bend microwave were plugged into a relocatable power tap. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.      g)     Tribal Lottery Systems Room 114:  Two relocatable power taps were plugged into an extension cord.  One relocatable power tap was plugged into another relocatable power tap.  The relocatable power taps were powering a Weller brand soldering station, Motorola battery chargers and DeWalt battery charger. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  a)  South Garage Shop:  Panel S1A,  cases of water and sheetrock were stored beneath the breaker panel.  Panel S1D, Deicer and boots were stored underneath the breaker panel. CORRECTED DURING INSPECTION  b)  Electrician/ Carpenter Shop:  Panel N1A, buckets of poly lime and metal electrical parts were stored underneath the breaker panel. Panel N1D, a metal step ladder and spools of wire were stored underneath the breaker panel.  c)   Ballroom Dishwashing Area:  Boxes of gloves and a trash can were stored underneath the breaker panels.  CORRECTED DURING INSPECTION  d)   Paradise Deli Kitchen:  Bins and pizza boxes were stored underneath the breaker panel.  e)   Tatoosh Restaurant Electrical Room:  An oil disposal system and caution/slippery floor signs were stored underneath a breaker panel.
