The powered industrial vehicle was used to hoist, transport and set wall panels and pre-fabricated trusses using synthetic web straps as rigged to the forks of the powered industrial vehicle, thereby exposing employees to struck-by and crushing hazards.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
