On 11/23/2015, workers from Reichardt Restoration LLC were installing shingles on a roof of a residential building located on 102 Fieldstone Estates Drive in Wentzville, MO. Three workers were not wearing eye protection while using pneumatic nail guns.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On 11/23/2015, workers from Reichardt Restoration LLC were installing shingles on a roof of a residential building located on 102 Fieldstone Estates Drive in Wentzville, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. Five workers were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.
