29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) Assembly Area - An employee used an Amada HFA 400w horizontal saw to cut metal bar stock to size. A 2 1/2 inch section of the blade was not guarded, thereby exposing the employee to a serious hazard, observed on or about 12-01-15.    Abatement certification required.

29 CFR 1910.303(b)(1)(i): 1910.303(b)(1)(i) - Electric equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees. In that, the employer's failure to consider the suitability of electrical outlet installation exposed employees to a workplace fire hazard:   a)  Near machine # 102-VF  -  An employee workbench had a hanging flexible electrical cord that supplied power to a lamp on the bench through a 4 receptacle outlet box that had knockouts on the box and was not designed for that type of use. The employee was exposed to an electrical shock hazard, observed on or about 12-01-15.   Abatement certification required.
