29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: a) On or about November 7, 2015 the employer did not ensure employees unplugged the Pot-Devin glue applicator from the electrical outlet prior to the removing glue pans from the back-side of the machine, exposing employee(s) to rotating parts from the glue rollers. b) On or about November 7, 2015 the employer did not ensure employees unplugged the Pot-Devin glue applicator from the electrical outlet prior to using a hand tool to remove glue from the rotating roller, exposing employee(s) to in-running nip point hazards.

The employer received confirmation on November 10, 2015 that the injury involved an amputation.
