29 CFR 1926.453(b)(2)(v):   A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift.  On 12/01/2015, an employee from Bruces Electric Inc. was working on a sign in front of the OFallon Center strip mall located at 1300 Highway K, OFallon MO.  The employee was installing light bulbs in a sign from an Elliot extensible and articulating platform, model M48-R / serial # 006277.  The Elliot platform was mounted on a Ford F-550 Heavy Duty Truck.  The employee was not wearing a body belt with a lanyard attached to the boom or basket while working from the extensible and articulating platform.
