The most recent occurrence of this took place at the job site located at 2810 Heartland Road, Lexington, NE where the employer had two employees of the company sheeting a pre-engineered steel building while working out of an aerial lift 14 feet 6 inches above the ground without wearing a fall arrest harness and a lanyard tethered to the connection point inside the aerial lift basket.
