29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146:     a) Kinetic Hydrovac Services, LLC dba Kinetic Hydrovac at 1189 Quentine, Milliken, CO 80543: On or before December 3, 2015, the employer did not ensure that employees serving as entrants and/or attendees while conducting work inside permit required confined spaces, which included both biofuels pits and frac tanks, had received appropriate training.  The employer failed to provide appropriate training to ensure employees acquired the understanding, knowledge, and skills necessary to work safely in permit required confined spaces.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a) Kinetic Hydrovac Services, LLC dba Kinetic Hydrovac at 1189 Quentine, Milliken, CO 80543: On or before December 3, 2015, the employer did not ensure that employees serving as entrants and/or attendees while conducting work inside permit required confined spaces, which included both biofuels pits and frac tanks, had received appropriate hazard communication training.  The employer failed to provide appropriate training to ensure employees at the time of their initial assignment.
