29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  In fabrication area, on or about December 1, 2015, and at times prior thereto, employees were exposed to a caught-in hazard, while operating an Amada Hydraulic Punch Press (Model: 36717) that had a guard removed around the torrent area.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) Fabricating area, on or about December 1, 2015, and at times prior thereto, employees were exposed to a stuck-by hazard, while operating a Bridgeport Manual Mill (Model: 2831237) with a rotating blade to cut and fabricate metal parts without a guard in place.  (b) Fabricating area, on or about December 1, 2015, and at times prior thereto,employees were exposed to a struck-by hazard, while operating a Burgmaster Drill Press (Model: A101018 Serial: 6165) to drill holes in metal pieces without a guard in place.

29 CFR 1910.305(a)(2)(i): Temporary electrical power and lighting installations rated 600 volts, nominal, or less were used for purposes other than those permitted in subparagraphs (A), (B) and (C) of this paragraph:  Fabrication area near band saw, on or about December 1, 2015, and at times prior thereto, employees were exposed to electrical shock hazards, while operating a Roll-N Band saw that had an extension cord (120 volt, 20 ampere) used as permanent wiring for the machine.
