29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Fabricating area, on or about December 1, 2015, and at times prior thereto, employees were exposed to a stuck-by hazard, while operating a Bridgeport Manual Mill (Model: 2831237) with a rotating blade to cut and fabricate metal parts without a guard in place. (b) Fabricating area, on or about December 1, 2015, and at times prior thereto,employees were exposed to a struck-by hazard, while operating a Burgmaster Drill Press (Model: A101018 Serial: 6165) to drill holes in metal pieces without a guard in place.

29 CFR 1910.305(a)(2)(i): Temporary electrical power and lighting installations rated 600 volts, nominal, or less were used for purposes other than those permitted in subparagraphs (A), (B) and (C) of this paragraph: Fabrication area near band saw, on or about December 1, 2015, and at times prior thereto, employees were exposed to electrical shock hazards, while operating a Roll-N Band saw that had an extension cord (120 volt, 20 ampere) used as permanent wiring for the machine.
