29 CFR 1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement: WORKSITE: The upper section of the Werner Model D 6224-2, 24-foot fiberglass extension ladder, on which the adjustable aluminum scaffold plank was placed, was not secured or braced to prevent accidental displacement. On or about 11/3/2016 while working on the above supported scaffold, the above ladder became displaced, causing the scaffold and the employee to fall more than 9 feet to the floor, and the employee sustained an injury from the incident.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. WORKSITE: The supported scaffold from which an employee was working was not provided with a proper access onto the scaffold.
