29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:   On December 2, 2015 employees were not protected from shock/electrocution hazards while changing energized parking lot lighting, exposing the employee to the hazards associated with contacting 208 volts.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:   On December 2, 2015 employees were not protected from a fall while working from truck mounted aerial platforms.   a) An employee working from Illinois plate number 391039 D bucket truck was not protected from a fall, exposing the employee to a fall of approximately 20 feet.   b) An employee working from Illinois plate number 120588 H bucket truck was not protected from a fall, exposing the employee to a fall of approximately 20 feet.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
