29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use:    a) For the establishment at 701 Victoria St., Brandon, FL 33510, as observed on or about 12/02/2015, the employer required employees to wear a 3M half-face tight fitting respirator with cartridges for job tasks such as installing 5 gall drums of laundry chemicals. A respiratory protection program had not been developed for the site location.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    a) For the establishment at 701 Victoria St., Brandon, FL 33510, as observed on or about 12/02/2015, employees were required to wear a 3M half face cartridge respirator while performing tasks such as changing out 5 gallon drums/buckets of laundry cleaning chemicals. Medical evaluations to determine the employee's ability to use the respirator had not been done prior to employees being required to use the respirator.

The employee was hospitalized.  The employer never reported the incident to OSHA.
