Employees were exposed to a struck-by hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system.   The employer did not ensure that the employees performing roofing operations at 2510 W Central, El Dorado, Kansas were protected from falling. Employees engaged in roofing activities from a 4:12 pitch roof were exposed to a fall hazard in excess of twelve (12) feet eave to ground.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:  The employer did not ensure that the employees performing roofing operations at 2510 W Central, El Dorado, Kansas maintain at least one hand on the ladder at all times when carrying and raising sheets of metal roofing and metal ridge caps up aluminum extension ladders. Employees were exposed to a fall hazards and struck-by hazards.

Employees were exposed to a fall and/or struck-by hazards.
