29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  Greg Allen dba Greg Allen Roofing and Construction, LLC had an employee exposed to struck-by hazards on the head, while cleaning up on the ground under employees working from the roof, on a residential home in Mahomet, Illinois.   The employer did not provide employee safety training in order to provide the employees with the knowledge to recognize construction safety hazards, as well as the procedures to follow in order to minimize or eliminate those safety hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    Greg Allen dba Greg Allen Roofing and Construction, LLC had an employee exposed to fall hazards without fall protection, while roofing a residential home in Mahomet, Illinois.   These fall hazards included:    a. 9 ft. from front side of house and  b. 15 ft. from rear of house.  Greg Allen dba Greg Allen Roofing and Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 411402, citation number 1, item number 3A and was affirmed as a final order on August 10, 2012, with respect to a workplace located at 2402 Waterbury Place, Champaign, IL 61820.  Greg Allen dba Greg Allen Roofing and Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 903903, citation number 1, item number 2 and was affirmed as a final order on September 12, 2014, with respect to a workplace located at 5016 Chestnut Grove, Lot 210, Champaign, IL 61822.  Greg Allen dba Greg Allen Roofing and Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 948692, citation number 1, item number 2A and was affirmed as a final order on March 28, 2014, with respect to a workplace located at 1838 Country 500 E, Champaign, IL 61820.
