29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment:  IN WASH YARD:  On or about December 1, 2015, and at times prior thereto, employees were exposed to chemical splash hazards to the body and face, while entering trucks and tanks for cleaning operations that were contaminated with waste from oil and gas well hydraulic fracturing operations, without being trained on the use of required personal protective equipment such as, but not limited to, FR suits, safety glasses, and face shields.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  IN WASH YARD:  On or about December 1, 2015, and at times prior thereto, employees were exposed to potential oxygen deficient atmospheres and/or inhalation of toxic gases or vapors, when required to wear tight-fitting 3M 6000 half-facepiece respirators with Organic Vapor/Acid Gas/P100 cartridges without the protections afforded by a respiratory protection program.  Employees routinely entered trucks and tanks for cleaning operations that were contaminated with waste from oil and gas well hydraulic fracturing operations.

29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146:  IN WASH YARD:  On or about December 1, 2015, and in times prior thereto, employees were exposed to chemical splash hazards, without being trained on how to use and maintain personal protective equipment, when performing cleaning operations for trucks and tanks that were contaminated with waste from oil and gas well hydraulic fracturing operations.

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:  IN WASH YARD: On or about December 1, 2015 employees were exposed potential oxygen deficient atmospheres and/or inhalation of toxic gases or vapors, while entering permit required confined spaces without being trained to render first aid when there was no hospital close to the site prior to allowing employees entry.
