29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about 12/1/15 located at 4030 Cameron Ct., Cumming, GA: Employees were framing the roof of a new house with lumber and ply wood as 2x8s were observed being swung overhead to the roof area.  2x8s scrap wood was observed being thrown to the ground as an employee was not provided a hard hat. Employees were without hard hats exposing them to a struck by hazards.

On or about 12/1/15 located at 4030 Cameron Ct., Cumming, GA:  Employees were observed framing a house while using a pneumatic nail gun and the skill saw without safety glasses.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  On or about 12/1/15 located at 4030 Cameron Ct., Cumming, GA:  Employees that were operating the skill saws used a piece of wood to wedge and hold the guard for the blade in the open position, while cutting 2x4s and plywood.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 12/1/15 located at 4030 Cameron Ct., Cumming, GA: Employees were framing the 8/12 pitch roof, installing 2x8 rafters and ply wood on to the roof of the 3rd floor while working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately up to 45 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 12/1/15 located at 4030 Cameron Ct., Cumming, GA: Employees were exposed to fall hazards, while performing roofing work, employees were using a portable extension ladder to gain access to the 2nd floor, and the ladder was 2 inches below the landing surface.
