29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Worksite located at 28800 Ida Street, Valley, NE; Irrigation Area: Employees are not always using lockout/tag out when entering the danger zones associated with hydraulic presses exposing them to amputation hazards. In this case, employees performing die changes are relying on the guarding interlocks rather than removing all energy from the machine using a viable lockout procedure and affixing a lockout device to maintain a zero energy state. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment Worksite located at 28800 Ida Street, Valley, NE; Irrigation Area: Personnel performing die changing operations are not ensuring that each exposed party is applying a personal lockout device which exposes them to amputation hazards. In this case, where two persons are involved in performing a die change where they enter the danger zone of the machine, a lockout device was not applied for each party nor was a comparable mechanism utilized. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.
