29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  a)  On or about December 2, 2015, and at times prior, employees were exposed to fall hazards of greater than 13 feet when they stood on the trash which was loaded in their open top semi truck trailer in order to position a tarp to cover the load.  The employer did not ensure that these employees utilized adequate personal protective equipment that would prevent them from falling. 29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  a)  On or about December 2, 2015, the employees were exposed to struck by hazards and/or crushing hazards in that the employer did not take the Caterpillar forklift out of service when the tires for the forktruck were in need of replacement.    b)  On or about December 2, 2015, employees were exposed to struck by hazards and/or crushing hazards in that the employer did not take the Caterpillar forklift out of service when the back up alarm failed to sound.
