29 CFR 1910.27(d)(3): "Ladder extensions." The side rails of fixed ladder were not extending 3 1/2 feet above parapets and landings. On or about November 25, 2015, employer did not ensure that the fix ladder extended a minimum of 3 1/2 feet above the mezzanine's landing. This condition exposed employees to fall hazards exceeding 4 feet.

29 CFR 1910.27(d)(4): Vertical grab bars did not have the same spacing as the ladder side rails: On or about November 25, 2015, employer did not ensure that the mezzanine's vertical guard rails section that were being used to climb on to the mezzanine's roof had the same spacing as the fix ladder side rails. This condition exposed employees to fall hazards exceeding 4 feet.
