29 CFR 1910.263(f)(3): The back(s) of divider(s) were not completely covered or enclosed to guard against the hazard of moving parts: a) At Toufayan Bakeries in Orlando, FL, an employee bypassed a guard on a WP Winkler WBD100 - Bagel Divider, the rear of the machine was open during operation exposing employee to moving parts. This violation was discovered on December 9, 2015.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) An employee's finger was amputated and he was taken to the hospital on October 24, 2015. Employer was officially notified of the amputation with the hospital notes and opened the worker comp claim on October 28, 2015. The employer did not notify OSHA. Springfield, MA OSHA office was informed of the amputation from Massachusetts Dept of Public Health on November 23, 2015.
